موضوع تحفة للصيادلة قبل او بعد الطعام ؟؟؟؟؟؟؟؟؟؟؟؟؟؟ الغذاء وتاثيره على الدواء......

كثيرا ما نواجه وخصوصا في مجال عملنا سواء بالمستشفى ام بالصيدلية الخاص السؤال الشائع لدى المرضى هل اخذ هذا الدواء قبل ام بعد الطعام ؟ اذن حينما يال المريض هذا السؤال هو يدرك فعلا ان للطعام تاثير على الدواء .......... وطبعا الطعام قد يؤثر على فعالية الدواء قد يؤثر على امتصاص الدواء ........ قد يؤثر على ظهور او عدم ظهور الاثار الجانبية للدواء ........... فلكي نكون على ادراك واسع عن هل ياخذ هذا الدواء قبل ام بعد الطعام في كل مرة سانزل مجموعة من الادوية واقول ناخذها قبل ام بعد والان سابدا مع الادوية التي تعمل على gastrointestinal systemes :-
a- h2 antagonists _ without regards to meal
b- anticholinergic _30 minuts before meal
c- metochlopramide_ 30minuts before meal
d- sucralafte _on empety stomach and at bed time
e- omeprazole _ immeditaly before meal

f- mebeverine hcl: taken 20 minuts before food to decrease gastrointestinal disturbance which is seen as side effect of the drug.
g- lanzoprazole: taken 30 minutes before food because food affect the bioviability of the drug .
h- lactulose: may be mixed with milk or fruitjuice to increase palatability.
j- bisachodyl: may be not taken with milk or antiacids because it is enteric coated tablets.
k-sulfasalazine,osalazine: may be take with food or immeditaly after food to decrease gastrointestinal upset.

Antimicrobial drugs:-
a-tetracycline _ take one hour before meal aor two our after because food and som diary products decrease its absorption

b- doxy cycline _ not affected by food
c- chlormphenicol_ itis the best to take it on empety stomach or take it with food to prevent gastointestinal upset.
d- rifampicine _ take it on empety stomach because food decrease itis absorption.

e- clarithromycin_not regard to meal.


f-clindamycin_ taken with meal and with full glass of water and on stand up position to decrease irritation of esophagus.
g- nitrofurantoin_ taken on empety stomach but if gastrointestinal upset occure can be take with food .
h- ciprofloxacilin_ take with meal or on after meal by two houre but diary products decrease itis absorption .
i- lincomycin _ take on empety stomach becaues food decrease itis absorption



h-norfloxacin_taken on empety stomach because food decrease itis absoption.
i-trimetheprim_ take after food by two hour or on empety stomach but if gastointestinal upset occure take it with food .
j-enoxacin_ take it on empety stomach because food decrease it is absorption .
k-pencillin_ the most of these drug groups affected by food and afood decrease it is absorption ,there are it is best to take it on empety stomach.but amoxcillin capsules may be take without regard to meal .


l- erthyromycin_taken on empety stomach or if gastrointestinal upset occure take it with food .
m-azithromycin_ take on empety stomach because afood decrease it is absorption .
n- naldixic acid _ take it on empety stomach and can be take with food if gasrointestinal upset occure.
o-cephalosporin_ may be take on empety stomach or with food if gastrointestinal upset occure but cefpodoxime and cefuroxime may be taken with food to increase absorption .

drugs act on cardiovascular system
1- quinidine _ take this drug with food to decrease gastrointestinal upset.
2-nifedipine_ take it without regard to meal
3-amlodipine_take with out regard to meal .
4-hydralazine _ take withfood to increase itis absorption
5-nodalol_ without regard to meal .

bisoprolol:_ not affected by food
mexiletine ;_taken with food to decrease gastro intestinal side effects.
propranolol:_ taken with food because food increase bioviailability
felodipine:_ not affected by food
